Medication decisions are made under psychiatric supervision and integrated into a broader therapeutic plan alongside individual counseling, group therapy and relapse prevention.

Common Questions About MAT

MAT is an evidence-based approach that combines FDA-approved medications with counseling and behavioral therapy to treat substance use disorders. The medications reduce cravings, ease withdrawal or block the rewarding effects of drugs and alcohol — creating enough stability for therapy to do its work. MAT is not a standalone solution, and simply taking medication without counseling isn’t MAT.
The medications used fall into a few categories. Agonists such as buprenorphine activate opioid receptors in a partial, controlled way, easing withdrawal and cravings without the intense high of illicit opioids. Antagonists such as naltrexone block opioid and alcohol receptors entirely, removing the rewarding effects of use. Anti-craving medications reduce the prolonged discomfort — anxiety, insomnia, restlessness — that often drives relapse in early recovery.

Buprenorphine is a partial opioid agonist used to treat opioid use disorder. It attaches to the same receptors as other opioids but activates them only partially, which reduces withdrawal symptoms and cravings without producing significant euphoria at therapeutic doses.

This combination is the active formulation in Suboxone, usually taken as a sublingual film or sublingual tablet that dissolves under the tongue. Buprenorphine manages withdrawal and cravings. Naloxone is included as a safeguard: taken as directed it has little effect, but it discourages misuse of the medication by injection. It comes in several strengths, and which is appropriate for a given person is a clinical decision made by a licensed provider.

Naltrexone is an opioid antagonist used to treat both opioid and alcohol use disorder. It blocks the effects of opioids and reduces the drive to drink. Because it’s a full blocker, it’s generally started after someone has fully withdrawn from opioids. Vivitrol — often referred to as the Vivitrol shot — is an extended-release injectable form of naltrexone given once monthly rather than daily.

No. This is a common belief and it’s both inaccurate and clinically harmful. Medications used in MAT are prescribed and monitored by licensed providers as part of a comprehensive treatment plan. Taken as directed, they stabilize brain chemistry rather than producing intoxication — which is what makes it possible to engage in therapy and rebuild daily life.

There’s no fixed timeline. Some people benefit from medication for months, others for considerably longer. Duration is a clinical decision made with your care team based on your progress and goals, and it can be revisited as your recovery develops.
